Investigation reveals alarming cockpit incident during 2024 passenger flight

A Lufthansa passenger flight operated without pilot supervision for approximately 10 minutes last year after the co-pilot lost consciousness while alone in the cockpit, according to a recently released investigation report. The incident, which occurred during a flight from Frankfurt to Seville on February 17, 2024, has raised serious questions about cockpit safety protocols and emergency procedures.

The German news agency dpa reported details from the official investigation conducted by Spain’s accident investigation authority CIAIAC, revealing that the Airbus A321 carrying 199 passengers and six crew members continued flying on autopilot while the co-pilot was incapacitated and the captain was unable to regain access to the cockpit.

Captain locked out during medical emergency

The investigation found that the medical emergency occurred while the captain was using the restroom, leaving the co-pilot alone in the cockpit. When the captain attempted to return, he found himself unable to regain entry despite multiple attempts to use the standard door opening code, which triggers a buzzer in the cockpit for the co-pilot to respond.

According to the report, the captain made five unsuccessful attempts to enter the cockpit using the standard procedure. A flight attendant also tried to contact the co-pilot via the onboard telephone system without success. During this time, the voice recorder captured unusual sounds from the cockpit that investigators determined were “consistent with an acute health emergency.”

Autopilot maintained stable flight despite unconscious co-pilot

The report indicates that while unconscious, the co-pilot apparently made unintentional contact with some flight controls. However, the aircraft’s autopilot system remained engaged and maintained stable flight parameters throughout the incident.

“Although the unconscious co-pilot apparently operated controls unintentionally, the aircraft was able to continue flying in a stable manner thanks to the active autopilot,” dpa reported, citing details from the investigation.

Emergency entry code finally resolves situation

After exhausting standard entry procedures, the captain ultimately resorted to entering an emergency code that would have overridden the cockpit door lock. However, moments before the automatic override would have activated, the co-pilot, despite his medical distress, managed to open the door from inside the cockpit.

Upon regaining access and assessing the situation, the captain made the decision to divert the aircraft for an unscheduled landing in Madrid, where the co-pilot was immediately transported to a hospital for medical treatment.

Lufthansa acknowledges investigation

When contacted about the incident, Lufthansa confirmed awareness of the investigation report and stated that its internal flight safety department had conducted its own inquiry into the events. The airline has not publicly disclosed the findings of its internal investigation, according to dpa.

The incident highlights the complex balance between security measures implemented after the September 11, 2001 attacks—which include reinforced cockpit doors and strict access protocols—and the need for emergency access during unexpected medical situations.

Aviation safety experts have noted that while modern autopilot systems provide substantial redundancy, the incident underscores the importance of continuously evaluating cockpit safety procedures for all possible contingencies.
