After nearly two decades of infertility and repeated failed IVF attempts across multiple countries, a couple has finally achieved pregnancy, thanks to an innovative application of artificial intelligence in reproductive medicine.

The breakthrough came at Columbia University’s Fertility Center, where doctors used a newly developed AI-powered system to detect hidden sperm in a man diagnosed with azoospermia, a rare condition that causes extremely low or no sperm in semen. Traditional detection methods have failed for years. But this time, technology succeeded where humans couldn’t.


What Made This Fertility Case So Unique?

The couple had endured 18 years of unsuccessful fertility treatment, largely due to the male partner’s azoospermia—a condition that typically requires invasive surgery or hormone therapy for any chance of success.

Instead of resorting to these painful and often fruitless options, the couple opted for a high-tech solution offered by Columbia’s STAR (Sperm Tracking and Recovery) system, which uses AI and imaging algorithms to scan semen samples and find viable sperm, even when present in minute quantities.

The result? The system found three hidden sperm, which were then used to fertilize eggs via in vitro fertilization (IVF), leading to a successful pregnancy.


How Does the STAR System Work?

The STAR method is as futuristic as it sounds. Here’s a breakdown of how it operates:

  • The system is connected to a microscope fitted with high-speed cameras and AI-powered imaging software.
  • It scans the sample on a specially designed chip, analyzing over 8 million images in less than an hour.
  • The AI has been trained to detect sperm cells—even when they’re hard to distinguish from other cells or debris.
  • Once found, each sperm is isolated into a tiny droplet, allowing embryologists to recover it safely.

“It found 44 sperm in one hour—compared to zero found by skilled technicians after two days of searching,” said Dr. Zev Williams, director of the Columbia University Fertility Center and lead developer of the system.


Why This Matters for Male Infertility Treatment

The STAR system could redefine infertility care for men diagnosed with azoospermia. Until now, treatment typically involved:

  • Surgical sperm extraction
  • Hormone therapy
  • Or worst-case: Using donor sperm

These methods can be expensive, painful, and often ineffective. In contrast, the STAR system:

  • It is non-invasive
  • Yields faster results
  • Costs under $3,000 for the full sperm isolation and freezing process

Currently, the STAR method is exclusive to Columbia University, but Dr. Williams hopes to expand its availability to other fertility centers soon.

The STAR system is part of a growing trend of AI integration in reproductive health:

  • Embryo grading algorithms: Predict which embryos are most viable before implantation.
  • CHLOE system: Analyzes a woman’s eggs before freezing to assess long-term fertility potential.
  • Cycle prediction tools: Help optimize timing for egg retrieval or insemination based on personalized data.

These technologies are streamlining fertility treatments, boosting success rates, and helping both patients and clinicians make better-informed decisions.

As AI continues to prove its value in clinical settings, tools like STAR could become standard practice in fertility clinics worldwide. But for now, this case stands as a powerful symbol of how emerging tech can bring hope to couples who thought they’d run out of options.

“It’s not just a breakthrough in fertility. It’s a breakthrough in how we think about medicine and machine learning,” said one fertility expert not affiliated with the study.
